摘要
医疗卫生行业事关人民群众生命安全和身体健康，既是党委政府心系民生，着
力提高医疗服务水平的重要抓手，又是深化医药卫生体制改革，持续增进民生福祉
的重要载体。 年，新医改的正式启动。公立医院改革作为新医改的核心任务，
关系到人民群众切身利益。一直以来，城市公立医院在医疗服务供给中发挥了主体
作用，由于涉及面广、服务人群多、利益格局复杂，国家层面又缺乏城市公立医院
改革的顶层设计和具体路径，时至今日尚未形成全国范围内认可的城市公立医院改
革模式。探索研究城市公立医院改革模式、方法、路径，最终实现“人民群众得实
惠、医务人员受鼓舞、医疗机构得发展”目标具有极为现实而深远的意义
本文对公立医院改革相关理论进行分析阐述，以作者所在的成都市第一人民医
院为研究对象，在研究分析公立医院改革的背景和现状的基础上，对成都市第一人
民医院改革探索现状及面临的问题进行了系统分析。综合分析了国内城市公立医院
改革探索的几种模式及美国、英国、德国、新加坡、法国等国外公立医院改革特征，
简要分析了不同模式的特点和存在的不足，总结分析了国内外公立医院改革的
有益借鉴和启示。通过对成都市第一人民医院发展现状采用 SWOT 分析法，进一步明
确医院内部优势和劣势，机遇和威胁，结合公立医院改革的共性特征，从政府、卫
生行政部门、医院三方角度提出推进公立医院改革的保障措施和政策建议，既对成
都市第一人民医院改革实践给予针对引导，也为其他城市公立医院改革提供有益借
鉴

ABSTRACT
The health care industry is related to people's life safety and body health, an
important livelihood is the Party committee and government governance, an important
dimension of social economic development level. In , the CPC Central Committee
and State Council opinions on deepening medical and health system reform (in )
formally issued, marking the official launch of the new medical reform. The reform of
public hospitals as the core task of the new health care reform, related to the vital interests
of the masses of the people. All along, the city public hospital played a main role in the
supply of medical services, because it covers a wide range, service groups, a complex
pattern of interests, the national level and the lack of top-level design of reform of public
hospitals in the city and the specific path, today has not yet formed a nationwide
recognition of city public hospital reform model. Research on city public hospital reform
mode, method, path, and ultimately achieve tangible benefits to the people, encouraged
by the medical staff, medical institutions development target is of great realistic and
far-reaching significance.
This paper analyzes the related theory of public hospital reform, based on the author's
first people's Hospital of Chengdu City as the research object, based on the research
background and current situation of public hospital reform, the first people's Hospital of
Chengdu city reform status and problems were analyzed. A comprehensive analysis of the
several modes of reform of public hospitals in the city of domestic and American, Britain,
Germany, Singapore, France and other foreign public hospital reform characteristics,
A brief analysis of the characteristics of different patterns and problems, the analysis
of useful reference and Inspiration of the reform of public hospitals at home and abroad is
summarized. Based on the current situation of the development of the first people's
Hospital of Chengdu City by the method of SWOT analysis, to further clarify the hospital

第一章绪论
1.1 选题的由来
医疗卫生行业事关人民群众生命安全和身体健康，既是党委政府高度关注的民
生目标，也是人民群众共享改革发展成果的重要载体。，新医改的正式启动
作为新医改确定的五项重点改革任务，除了公立医院改革外的其他四项改革任务已
经在取得了阶段性成果，但作为重中之重的公立医院改革除了在县级及以下
公立医院进行改革试点外，在全国层面，城市公立医院特别是副省级城市公立医院
改革还处于“真空”地带。由于涉及面广、服务人群多、利益格局复杂，国家层面
又缺乏城市公立医院改革的顶层设计和具体路径，虽然部分地区进行了不少有益的
改革尝试，但大多只是在“点”上探索，在“线”上试点，时至今日尚未形成全国
范围内认可的统一改革模式。 年，国家层面选取了16个公立医院改革试点城市，
各省区市根据地域划分、经济发展水平、改革难易度等因素选取了1-2个公立医院改
革省级试点城市。成都市虽然不在国家试点城市之列，但主动适应新医改要求，在
体制改革、机制创新、能力提升、发展路径等方面进行了积极探索，成都市第一人
民医院作为成都市市级公立医院的排头兵，在公立医院改革方面进行了相关探索，
取得了一定的改革经验，为公立医院改革进入深水区提供有益的借鉴。
