I hereby state my intent to teach for EF after reading the contract, job description and code of conduct. By signing this document, I state that I am in good physical health and under no influence of drugs, alcohol or any other abusing substance or medication. This is only a provisional document and a formal work contract will be signed upon arrival at the designated school in China.
