I hereby state my intent to teach for EF after reading the contract, job description and code of conduct. By signing this document, I state that I am in good physical health and under no influence of drugs, alcohol or any other abusing substance or medication. This is only a provisional document and a formal work contract will be signed upon arrival at the designated school in China. If by any reason, I fail to sign the formal work contract or comply with the terms and conditions of the working contract, then I am liable for any incurred costs on EF China such as prepaid flights and accommodation costs, etc. At the same token, EF English First China will remunerate me at the minimum net monthly salary plus benefits as outlines in the contract of employment, providing that the working contract is fulfilled by me.
